In a press release Friday, Tammy MacDonald, 53, allegedly made the cards on June 20 and July 28, the U.S. Attorney’s Office for the District of South Carolina.

When asked about the cards in October, McDonald “lied, claiming that he did not have access to the Covid 19 vaccination record cards and never produced the wrong or incorrect vaccination card.” Authorities said.

Federal officials say McDonald’s personally filled in the cards for people he knew who had not been injected.

McDonald’s, director of nursing services at the Nursing and Rehabilitation Facility, was charged with two counts of fabricating a fake vaccination card and lying to federal investigators about his role in the card.

She was indicted on Friday on those charges and pleaded not guilty. He was given a 10,000 bond. If convicted, McDonald’s faces up to 15 years in prison for each count of presenting a fake card and five years for lying to federal authorities.

McDonald’s lawyer was not immediately available for comment.

Derek L. Jackson of the Office of the Inspector General of Health and Human Services said McDonald’s actions “deceived and endangered the public.”
